CONSULTANT PROPOSAL:

As a minimum your proposal shall include the: following:

A. An appreciation of the Scope of the project.

B. A schedule to indicate the approximate length of time ‘Tequired for completion of the
various phases of the project from the Notice to Comunence to the submission of the final
report, including any recommended milestone meetings throughout the project.

The curriculum vitae of personnel to be employed on this project. Please note that if your
firm is selected to undertake the project the City will only accept the nominated personnel
to work on the project.

The fee schedule of the various personnel for your firm who will be employed on this
project.

The total fixed consulting fees for the couspletion of the project.

Submit eight copies of the proposal.

THE CITY OF PORT COQUTILAM

The City will provide:
A. — Legal base maps of 1:2000 and 1:10,000 scale.

B. A complete set of reproducible base maps at a 1:12,500 scale of the sanitary sewer
network and the water network.

Contour maps with two metre intervals at a scale of 1:2000.
Air photos at a scale 1:2000 (1979).

Access and legal use of "SANSYS" program.

The City will hold an information meeting on October 17, 1991 at 10:00 a.m. in the third floor
Heritage Room, City Hall. | | :

Purpose of the meeting will be to answer general questions and provide further technical
information to consultants. The City will have available both Engineering and Public Works
staff at the meeting. . : |
