Transition Program

Coordinator /Facilitator
Full-Time (35 hours per week)

Job Description: Under the supervision of the Executive
Director, the incumbent will develop, coordinate, facili-
tale, and provide counseling support for the Transition
Program, which will assist and encourage participants to
prepare to enter the workforce or further thei education
and acquire new skills. Specific duties will be:

© Develop ond deliver curriculum for Personal
Development and Cross-Cultural programs.

© Work closely with NWCC instructor fo ensure delivery
of basic literacy and numeracy skills.

® Plan and lead occasional outings in consultation with
the group.

*Provide counseling support lo participants and help
them to develop individual action plans.

@ Keep client records and stats.

* Actively participate in case management/program
