NOTICE TO APPLICANT

This application form must be completed and signed in duplicate.
Five (5) copies of the Proposed supplement or new cariff must be submitted
with the application. '
Read carefully che provisions of Part 8 of the Regulations. Non-compliance
with the Regulations may result in refusal of the application. Note the
following points parcicularly;:- .
(a) Public notice of the application must be posted at least thirty days
prior to the proposed effective date.
(b) A notice of the application must state that the application is SUBJECT 10
THE CONSENT OF THE MOTOR CARRIER COMMISSION and that ANY OBJECTION MAY BE
FILED WITH THE SUPERINTENDENT OF MOTOR CARRIERS, Burnaby V5G 3X5, B. C., up
to a date specified in the notice, which shall be at least two weeks after
the date on which the notice is posted and at lease cen days prior to the
propoyed effective dace.
