If death occurs in such i 31 day period the life insurance coverage under the Plan will nevertheless be paid. \ : ‘13-REINSTATEMENT OF A -' COVERED EMPLOYEE b 5 A former covered employee who again becomes an eligible .; employee will be immediately reinstated in the Plan upon his return to work provided his return to work occurs not more than 18 months from the date he last worked as a covered } employee. * 14-CONTRIBUTIONS BY COVERED EMPLOYEES If a covered employee has coverage on the first working day of any month he will have deducted from his earnings for the first pay period in that month the sum of $2.45 as his regular ' contribution for his coverage in that month. a A covered employee who is placed on layoff and who is en- eo titled to continuation of coverage in accordance with Section : 10 will have deducted from his earnings for the pay period in / by which layoff commences the sum of $7.35 as an advance contri- ; bution for his coverage for the three calendar months next following. ie If he returns to active employment before layoff has extended for three months, any unused advance contribution will be a credit against regular contributions until exhausted. If layoff extends beyond three months and the covered em- ployee is entitled to continuation of coverage beyond three months of layoff in accordance with Section 10, his employer AND WELFARE PLAN will make a special contribution of $2.45 for each month of lay- off beyond three but not beyond six months and will deduct and retain an amount equal to any such special contributions made in respect of the covered employee from his earnings for the first pay period following his return to active employment. If the covered employee does not return to active employment with the employer the Plan, through administrative procedures or through direct reimbursement annually, will save the em- ployer harmless against loss through having made special con- tributions on behalf of such covered employee. 15-CONTRIBUTIONS BY EMPLOYERS Each employer of covered employees will contribute an amount equal to the regular, advance and special contributions in respect of his covered employees. 146-PAYMENT OF CONTRIBUTIONS | Once each month, each employer of covered employees will remit an amount equal to the regular, advance and special con- tributions for that month, together with the matching employer contribution to the Forest Industry Health and Welfare Plan Trust Fund in care of the Administrator of the Plan. Each em- ployer will be advised from time to time of the name and mail- ing address of the Administrator. 17-CLAIM PAYMENT PROCEDURES AND ADMINISTRATION Each employer will maintain supplies of claim forms at each location where covered employees are employed and will take such actions as are necessary to expedite the payment of claims. An administrative manual will be provided by the Trustees to assist each employer in implementing and maintaining simple and efficient procedures for claim payment and administration. 18-PLAN SUMMARY The Trustees will make available to each covered employee a Plan Summary outlining the major provisions of the Plan. Each covered employee will also receive a:Certificate of Insurance summarizing the major provisions of the group insurance con- tract. The Trustees may, from time to time, issue information bul- letins to supplement the information contained in the Plan Sum- mary and Certificate of Insurance. 19-INTERPRETATION The Trustees will decide all questions arising on matters of interpretation of the Plan and their decision will be binding on each eligible employee, covered employee, former covered employee and his or their employer. The Trustees may establish and may publish interpretive rulings from time to time for distribution to contributing em- ployers and/or covered employees. 20-AMENDMENT . The Trustees may amend this Plan at any time and from time to time, provided always that the Plan must not be in- consistent with the provisions of the Coast Master Agreement which relate to Health and Welfare.
