KENNEMETAL INC. - PERMIT APPLICATION TO AMEND PERMIT # 8201

SUMMARY OF EVENTS:

1. June 16/93 - approximately 11:00 a.m. City Administrator’s office contacted by
Linda Asgeirsson advising that an amending permit has been issued to
Kennemetal. Apparently there was no comments received from the City.

June 16/93 - approximately 11:30 a.m. Danielle Pagé passed information to J. E.
Yip and requested that this issue be investigated.

Approximately 11:45 p.m. - J.E. Yip contacted Linda Asgeirsson to review issue
and subsequently contacted Mr. Ian McDonald of the Ministry of Environment
Environmental Protection. Mr. McDonald referred the issue to Mr. Dave
Robertson as he was dealing with the application. Mr. Robertson was not in and
was to retum call.

Approximately 3:30 p.m. Mayor arrived at the City Hall and requested a meeting
with J. E. Yip to review this matter. Following discussion Mr. Yip was asked to
call Victoria.

Approximately 4:00 p.m. - Contacted Victoria, Ministry of Environment, Dr. J.
O’Riordan - Assistant Deputy Minister, Environmental Management Department
Reviewed the issue and confirmed that a permit application was made. He
advised that he was holding an approval letter until he receives verification from
the Regional Office that the local jurisdiction had responded. O'Riordan advised
that the Regional Office would be in contact with Mr. Yip.

June 17/93 approx. 8:30 a.m. - Councillor J.J. Keryluk contacted the City
Administrator and requested that a meeting for 9:00 a.m. be arranged to review
this issue.
