holder, all which form a part of this Pe-mit and constitute the terms and conditions of this Permit.

This permit is issued on the condition that the permit holder fully comply with all provisions of
the Port Coquitlam "Soil Removal Regulation Bylaw, 1993, No. 2827" and all terms and
conditions of this Permit.

This Soil Removal Permit is issued this day of , 19___ and shall
expire six months after the day of issuance.
