WESTERN CANADIAN LUMBER WORKER pe jessy) FOREST INDUSTRY HE |-EFFECTIVE DATE The Plan is effective on and after 1 October, 1961. 2-ELIGIBLE EMPLOYEES A Any employee within the bargaining unit of a coast forest products operation who is subject to the bargaining authority of Local Unions represented by Regional Council No. 1 of International Woodworkers of America and whose employer is a member of Forest Industrial Relations Limited is an elig- ible employee. “Employment” and “work” as used herein shall mean em- ployment and work as an eligible employee. 3-COVERED EMPLOYEES : (A) Any eligible employee who, on 1 October, 1961, is (i) actively at work, or : (ii) absent on Workmen’s Compensation claim for tem- porary disability, or (iii) absent on weekly income claim under any group in- surance plan existing on 30 September, 1961 in coast forest products operations, or . (iv) absent through sickness or non-occupational acci- dent (absence to have commenced on or after 1 Sep- tember, 1961) but not on weekly income claim under any group insurance plan existing on 30 September, 1961 in coast forest products operations, will become a covered employee on 1 October, 1961 and will remain a covered employee until his coverage ter- minates in accordance with the terms of the Plan. For purposes of (i) above, an eligible employee shall be deemed to be actively at work on 1 October, 1961 if that day is not a regularly scheduled work day for such employee, pro- vided the employee worked his last regularly scheduled work day prior to 1 October, 1961. (B) Any eligible employee who, on 1 October, 1961, is (i) absent on layoff, or (i) absent on personal leave of absence, or (iii) absent through sickness or non-occupational acci- dent (absence to have commenced before 1 Septem- ber, 1961) but not on weekly income claim under any group insurance plan existing on 30 September, 1961 in coast forest products operations, will become a covered employee upon his return to work, provided such return to work occurs before 1 April, 1963, and will remain a covered employee until his coverage terminates in accordance with the terms of the Plan. (C) Any eligible employee hired after 1 October, 1961 will become a covered employee if and when (i) three months have elapsed since his: first day of work, and, (ii) he is actively at work and will remain a covered employee until his coverage terminates in accordance with the terms of the Plan. Each covered employee will be required to complete an en- rolment form. ww 4-AMOUNTS OF COVERAGE | The Plan provides coverage in the following amounts: Life insurance $5,000 Accidental death and dismemberment insurance .. $5,000 Weekly indemnity insurance $35.00 Where payments are made in respect of a period or an occur- rence after 30 September 1961 as a result of a covered employ- ee’s participation in any other group insurance plan existing on 30 September 1961 in coast forest products operations, or in respect of an ordinary policy of insurance held as a result of a covered employee having exercised the conversion priv- ilege in accordance with Section 11, the liability of this Plan shall be discharged by the payment of such amounts as are necessary to provide, in total, the benefits of this Plan. 5-LIFE INSURANCE 4 6-LIFE INSURANCE The amount of life insurance coverage will be paid on the death of a covered employee from any cause. ON TOTAL DISABILITY 7-ACCIDENTAL DEATH AND If a covered employee becomes totally disabled, as defined in the group insurance contract, before attaining age 65, his life insurance coverage will remain in force without further premium payment for as long as he is so disabled. DISMEMBERMENT INSURANCE (A) The amount of accidental death coverage will be paid on the death of a covered employee through accidental means for which compensation is not payable under any Work- men’s Compensation Act or any future, similar legislation. (B) The amount of accidental dismemberment coverage will be paid, in whole or in part, on the loss of limb or sight by a covered employee through accidental means for which compensation is not payable under any Workmen’s Com- pensation Act or any future, similar legislation, on the basis of the following schedule: Loss of two hands $5,000 Loss of two feet 5,000 Loss of sight of two eyes 5,000 Loss of one hand and one foot 5,000 Loss of one hand and sight of one eye ounce. 5,000 Loss of one foot and sight of one Eye nn 5,000 Loss of one hand or one foot 2,500 Loss of sight of one eye 2,500 Exclusions and conditions of payment will be in accord- ance with the provisions of the group insurance contract. 8-WEEKLY INDEMNITY INSURANCE | The amount of weekly indemnity coverage will be paid to a covered employee who is necessarily absent from work be- cause of accident or sickness for which compensation is not payable under any Workmen’s Compensation Act or any future, similar legislation and who is under the care of a qualified practitioner as defined by the Trustees. Payments will be for the period commencing with the first day of any such absence due to accident or with the sixth day of any such absence due to sickness and will continue during the whole of any such absence, but not beyond a period of 26 weeks for any one absence, and will not be paid during a period of regular holi- days with pay taken by the covered employee in accordance with the terms of the Annual Holidays Act and the Coast Master Agreement. Absences separated by a return to work of less than two weeks will be considered one continuous ab- sence if for the same or related causes. No benefit will be paid for absences due to pregnancy or complications thereof. 9-UNDERWRITING | A All coverage will be provided under the terms of a group insurance contract or contracts entered into by the Trustees and an insurance company licensed to transact business in Brit- ish Columbia. Should any conflict arise between the terms of the group insurance contract or contracts and the terms of the Plan, the terms of the Plan shall prevail.
