June 17/93 approximately 8:30 a.m. - Mr. Jim McCracken called Mr. J. Yip to
advise that:

a) Initial application was made by Kennemetal in Dec. 1992.

b) Lerters forwarded to various agencies in early January, 1993. (G.V.R.D.,
Health Unit. City of Port Coquitlam, Water Resources. etc.)

c) No comments received apart from Health: Department; therefore assumed no
concem.

d) Permit approval granted March 31/93

e) Ontario Government has requested a letter from the Provincial Government
confirming acceptance of the material. It was noted that tr 3 was the letter
presently held by Dr. O’Riordan.

June 17/93 approx. 9:00 a.m. - Meeting to review this issue was heid in the City
‘Administrator's Office with Councillor Keryluk, Bryan Kirk and Jeff Yip. Mr.
Yip was requested to secure copies of the following documents;

a) M.O.E. request to City for comments on the application;

b) Initial application from Kennemetal;

c) Approved permit:

d) Draft copy of letter to the Ontario government.

Approximately 10:15 a.m. telephone call to Mayor requesting that he call the
Hon. J. Cashore (Minister of Environment, to request that the proposed letter to
Ontaric Government be put on hold until this issue is reviewed further.

June 17/93 approx. 10:30 a.m. Mr. J. Yip called Mr. McCracken to request the
information (follow up by fax). ,

June /93 approx. 11:00 a.m. - Advised by Danielle Pagé that the Mayor had
calle advise that he was in contact with the Minister an¢that the Minister has
instr. :ed that information pertaining to this issue be forwarded to the Mayor’s
office this afternoon.

Councillor Keryluk was advised of this information by the Administrator’s Office
