Enclosed, in triplicate, is Agreement covering the above mentioned
installation. Would you kindly have this Agreement completed
as follows:

1. A signing officer’s initials on the plan in the stamped
box which reads "This is the Plan Referred ....." is provided for
this purpose. |

2. The corporate seal and authorized Signatures
on the last page, or a witnessed ‘signature of the Applicant,
if not a Corporation.

Upon completion, please return all copies to this office for
execution by Canadian Pacific.

One fully executed copy will be returned to you, in due course,
for your records. Please note that no work is to be undertaken until
these documents have been executed by Canadian Pacific and returned.

The Corporation of the City of Port Coquitlam will be billed by our
Manager, Property Accounts, Montreal, for documentation fee $80.00,
plus first year rental of $10.00.
